After you submit your application

    After you submit your initial application, you will work with a representative of the NYC Department of Health and Mental Hygiene, Office of Radiological Health (ORH), to complete the application process. You may contact ORH by calling (718) 310-2840.

      Once your application is reviewed and approved, you will receive an invoice in the mail for the license fee. You must pay the license fee before your permit is issued.
